Flemingdon Health Centre is a community health centre that has been serving the community and health needs of

local residents since 1975. Our satellite, Fairview Community Health, will open in 2009 to serve residents in the

Don Mills area.

Our approach to community health encompasses the broad determinants of health such as education, employment,

isolation, food security and social supports, and utilizes a community development model to promote health, prevent

disease, and build community capacity.

Purpose: The social worker will work in a multi-disciplinary team and will take the leadership in identifying,

assessing and addressing/assisting with the clinical and psycho-social needs of clients of the satellite, within the

framework of the determinants of health with an emphasis on prevention and health promotion. As well, the social

worker will work with community groups and individuals to promote and assist in the implementation of health

programs using a community development approach.

Educational and personal requirements:

• 3-5 years experience in a community based health or human service organization located in a culturally diverse

setting

• MSW and eligibility for registration with the Ontario College of Social Workers and Social Services Workers

• Knowledge and proficiency in assessment and short term counselling

• Knowledge and proficiency in culturally appropriate counselling and social work services

• Knowledge of and commitment to anti-oppression and anti-racist social work theory and practice

• Knowledge of community resources, income security programs, food security resources, and subsidized housing

sectors

• Experience working with economically and socially marginalized populations

• Experience and knowledge in social work practice with groups and communities

• Knowledge and experience in community development, social action, health promotion, and community outreach

• Ability to deal effectively with crisis

• Excellent team work, communication, interpersonal, and organizational skills

• Experience in successful grant / proposal writing

• Research skills, particularly in community-based research

• Ability to work at satellite and other locations, as well as flexible hours including some evenings

• Ability to speak other languages, preferably Farsi, Pashto, Arabic or Mandarin, is an asset

Responsibilities:

• Provide psychosocial intake assessment and short-term counselling services to individuals, couples, and families

• Provide information, referrals, assistance and advocacy; specifically in housing, income security, food security, and

settlement

• Provide crisis counselling, referrals and transitional support for women and children leaving abuse

• Facilitate client linkages with internal and external resources and supports

• Participate in the coordination of shared client care with an interdisciplinary team of health service providers

• Communicate with and coordinate client services with staff of external agencies and organizations

• Participate in planning and implementing group programs, self-sustainable community initiatives, and health

education/promotion programs

• Conduct workshops on social issues for health centre programs

• Participate in community-based groups or committees relating to social determinants of health

• Participate in or initiate research in areas related to the social determinants of health

• Travel to locations within the satellite’s catchment area will be required

• Other activities as assigned by the Satellite Coordinator
